Job summary

Creative Solutions In HealthCare is seeking a Per Diem (PRN) Physical Therapist to join our in-house rehab team serving residents with person-centered care. You will assess patients, develop and implement treatment plans, and collaborate with the interdisciplinary team to achieve positive outcomes while providing education to patients and families.

Texas license in Physical Therapy is required; SNF/long-term care experience preferred.

Qualifications

• Active Texas license in Physical Therapy and in good standing.
• Proven ability to apply current physical therapy practices.

Responsibilities

• Consult with patients to assess physical condition.
• Assess evaluations and tests and interpret results.
• Develop and implement treatment plans.
• Create personalized rehabilitation programs.
• Administer prescribed PT treatments per plan of care.
• Ensure documentation complies with regulations and plan of care.
• Monitor patient progress and adjust treatment as needed.
• Provide education to patients, family, and caregivers.
• Collaborate with the interdisciplinary team for quality care.
• Attend required rehab department meetings.

Job description

We have an amazing opportunity for a Per Diem (PRN) Physical Therapist! We are looking for a PT to join our in-house team of outstanding professionals that service our residents through person-centered care to achieve positive outcomes!

Physical Therapist Responsibilities

• Consulting with patients to learn about their physical condition.
• Assessing and interpreting patient evaluations and test results.
• Developing treatment plans using a variety of treatment techniques.
• Creating personalized rehabilitation programs for patients.
• Administering medically prescribed physical therapy treatments to relieve pain, improve or restore function, and promote healing.
• Ensure all treatment delivered is in accordance with the established plan of care.
• Monitor patient response to treatment intervention and modify treatment approach and directive as clinically appropriate with IDT collaboration.
• Provide clinical support and education to patients, family members, and caregivers.
• Collaborate with other members of the interdisciplinary team to ensure quality patient care is provided.
• Ensure documentation is completed thoroughly and timely and complies with all company and state/federal regulatory requirements.
• Attend required meetings as designated by the Director of Rehab.

Physical Therapist Skills

• SNF/Long term care experience preferred.
• Current knowledge of treatment practices
• Knowledge of Net Health Electronic Documentation System preferred.
• Ability to manage patients with different types of personalities.

Physical Therapist Requirements

• Active/Valid Texas license in Physical Therapy
• In good standing with all regulatory agencies and licensing boards
