Continuum Rehab Group is seeking a Physical Therapist to join our in-house therapy team serving Skilled Nursing/Long Term Care Facilities. We offer a comprehensive compensation package including a sign-on bonus and benefits.

Experience/Qualifications:

Must have graduated from an accredited University and have valid (or in process) License in appropriate state.

Some duties for this position include:

• Provides rehabilitative, skilled and medically necessary treatment intervention to patients with physical functioning disorders per regulatory and clinical practice requirements.
• Provides skilled therapy services to patients who need rehabilitation services.
• Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to physical disorders.
• Supervise Physical Therapy Assistants, temporary licensees, students, and support personnel in accordance with state licensure requirements and professional standards
