Job summary

Continuum Rehab Group is seeking a Physical Therapy Assistant to join our in-house therapy team serving Skilled Nursing/Long Term Care facilities in Prescott Valley, AZ. You will collaborate with licensed therapists to deliver evidenced-based rehabilitation and support residents in achieving optimal recovery and quality of life.

This position offers a competitive pay range ($34-$40/hour), a comprehensive benefits package including 401k, PTO, medical, dental and vision, free CEUs through CEU360,

Qualifications

Responsibilities

• Utilizes evidence-based practice to support clinical interventions.
• Provides rehabilitative, skilled and medically necessary treatment under supervision of a Physical Therapist.
• Provides skilled therapy services to patients needing rehabilitation.
• Assesses and documents patient progress and communicates with supervising Physical Therapist to adjust treatments.
• Provides counseling to patients and families.
• Contributes to discharge summaries per regulatory and clinical requirements.

This position is open to therapists at all experience levels.

Some duties for this position include:

• Utilizes evidence-based practice to support clinical interventions.

• Provides rehabilitative, skilled and medically necessary treatment intervention to patients with physical functioning disorders per regulatory and clinical practice requirements under the supervision and direction of an Physical Therapist.

• Provides skilled therapy services to patients who need rehabilitation services.

• Assesses and document patient's progress and communicates with supervising Physical Therapist to adjust treatments accordingly.

• Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to physical disorders.

• Provides input to supervisor for discharge summary of services provided per regulatory and clinical requirements.
