Job summary

Continuum Rehab Group is seeking a Licensed Physical Therapy Assistant to join our in-house therapy team serving Skilled Nursing/Long Term Care Facilities in Prescott Valley, AZ.

You will provide evidence-based rehabilitative and skilled therapy under the supervision of a Physical Therapist, participate in program development, document progress, and contribute to discharge planning while delivering compassionate care and quality outcomes to residents.

Qualifications

• Must be Licensed PTA in the state of Arizona.
• Ability to implement evidence-based rehabilitation techniques.
• Strong documentation and communication with the care team.

Responsibilities

• Uses evidence-based practice to support interventions.
• Provides skilled rehabilitative therapy under supervision of a PT.

Experience/Qualifications:

Licensed Physical Therapy Assistant

This position is open to therapists at all experience levels.

Some duties for this position include:

• Utilizes evidence-based practice to support clinical interventions.
• Provides rehabilitative, skilled and medically necessary treatment intervention to patients with physical functioning disorders per regulatory and clinical practice requirements under the supervision and direction of an Physical Therapist.
• Provides skilled therapy services to patients who need rehabilitation services.
• Assesses and document patient’s progress and communicates with supervising Physical Therapist to adjust treatments accordingly.
• Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to physical disorders.
• Provides input to supervisor for discharge summary of services provided per regulatory and clinical requirements.
