Qualifications

• Graduate of an approved curriculum in Physical Therapy.
• Is in good standing with all regulatory agencies and licensing boards.
• Current, active license as Physical Therapist, and is able to practice unencumbered.
• CPR Certification, as required for license renewal.
• Experience in long term care preferred.

Responsibilities

• Evaluates and treats patients per physician orders and facility/payer source guidelines.
• Develops treatment plans and provides therapy to achieve best functional outcomes.
• Educates staff and third party payers on benefits and outcomes of intervention.
• Completes documentation including evaluations and progress notes.
• Maintains EMR entries and submits billing documentation timely.
• Collaborates with rehab team to develop programs and improve utilization.
