Job summary

Lacamas Creek Post Acute is seeking a compassionate Physical Therapist (PT) to join our rehabilitation team. Full-Time: $40–$55 per hour; PRN: $60–$65 per hour.

You will evaluate patients, develop plans of care, and collaborate with a multidisciplinary team to restore strength and independence. We value compassionate, patient-centered care and offer a supportive environment with opportunities to grow skills and advance your career.

Qualifications

• Graduate from an accredited Physical Therapy program.
• Current Physical Therapist license or eligibility for licensure.
• Strong clinical, communication, and interpersonal skills.

Responsibilities

• Evaluate patients and develop individualized plans of care.
• Provide skilled physical therapy services to improve strength, mobility, balance, and function.
• Monitor progress and adjust treatment plans as needed.
• Educate patients and families on exercises and strategies to support progress.
• Document evaluations, treatments, progress, and outcomes.
• Collaborate with the care team and supervise assistants as appropriate.
