An innovative healthcare facility is seeking a dedicated Physical Therapist to join their team on a per diem basis. In this role, you will evaluate patients promptly, develop effective treatment plans, and collaborate closely with physicians and nursing staff. Your expertise will guide the training of support staff and ensure high-quality patient care in a supportive environment. Evaluate patients within twenty-four (24) hours or one business day of physician referral. If evaluation cannot be completed within 24 hours, document the cause for delay clearly in the medical record. Develop an effective treatment plan and obtain approval from the referring physician. Treat patients according to the physician’s treatment plan.
Assist the nursing department with training of Restorative Aides and supervise Physical Therapy Assistants in direct patient care and related activities, following state practice act guidelines. Communicate patient progress, issues, and plans with supervisors and other health team members.
Participate in Patient Care Conferences, Utilization Review meetings, and Rehabilitation Conferences as needed. Conduct in-service training for staff and record evaluations, treatment notes, progress notes, and recertification documentation in compliance with PT Board State Practice Act, governmental, and third-party payer requirements.
Instruct patients’ families or nursing staff in maintenance programs and caregiver training to prepare for discharge. Participate in discharge planning and recommend necessary durable medical equipment such as braces, walkers, canes, and grab bars to facilitate mobility and independence.
Report equipment issues to ensure maintenance. Maintain positive, effective relationships with patients, families, staff, and colleagues.
Supervise PT assistants, aides, and students.
Frequent standing, walking, reaching, talking, and hearing. Occasional sitting, climbing, and other physical activities. Ability to lift in excess of 20 pounds regularly. Work environment typically low to moderate noise. Reasonable accommodations available for disabilities.
