Job summary

KeystoneCare, a nonprofit provider of in-home healthcare services, is seeking a Physical Therapist. In this role, you'll help patients strengthen mobility and maintain independence through thoughtful, personalized care.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Pennsylvania Physical Therapy License.
• Minimum two years of physical therapy experience in Home Health or similar settings.
• Experience treating geriatric patients in a home health context.
• Valid Driver's License and reliable vehicle.

As a Physical Therapist at KeystoneCare, you will make a meaningful impact by helping patients strengthen mobility, restore function, and maintain independence through compassionate in-home care. Responsibilities

• Provide personalized rehabilitation services to patients in their homes.
• Assess, plan, and carry out treatment programs focused on improving mobility, strength, and overall functional abilities.
• Collaborate closely with a multidisciplinary team to support patient care and recovery.
• Guide patients through their recovery journey with compassion and professionalism.
• Help restore patients' independence and well-being through high-quality therapeutic care.

• Graduate of an accredited Physical Therapy program

• Current Pennsylvania Physical Therapy License
• Minimum two years of physical therapy experience in Home Health or similar settings
• Experience treating geriatric patients in a home health or similar care setting
• Experience with post-surgical rehabilitation, neurological conditions, and orthopedic rehabilitation in a home health or similar care setting
• Experience providing care in hospice-adjacent contexts
• Valid Driver's License and reliable vehicle
• Proof of Auto Insurance
• Strong communication skills for effective collaboration with patients, families, and the care team
• Excellent organizational abilities and experience managing a diverse caseload
• Eligible to work in the United States
• Experience with Matrix Care is a plus.
