Job Details

The Physical Therapist is responsible for the evaluation, treatment, and discharge planning of all patients referred for their services. The Physical Therapist is an essential healthcare provider committed to helping patients reach their highest level of function. The role requires collaboration with other healthcare professionals to coordinate treatment plans and care, and adherence to the mission, vision, and values of Jefferson Health.

Essential Functions

• Provide evaluations, treatment planning, interventions, and discharge planning for a full caseload across various settings including acute care, skilled facilities, rehab, and outpatient clinics.
• Design and implement treatment programs to aid recovery and rehabilitation.
• Administer safe therapeutic interventions and training for patients and families using appropriate tools and techniques.
• Document patient progress, treatments, evaluations, and discharge summaries accurately.
• Perform other duties as assigned.

Qualifications

Bachelor’s, Master’s, or Doctoral Degree from an accredited Physical Therapy Program.
Experience of 0-2 years in clinical practice; 1 year preferred.
Valid state license in Physical Therapy.
BLS and CPR certifications via AHA are required.
