Job Details

The Physical Therapist is responsible for the evaluation, treatment, and discharge planning of all patients referred for their services. The therapist is an integral healthcare provider accountable for assisting the patient to reach their highest level of function.
The therapist will uphold the mission, vision, and values of Jefferson Health.

Works in conjunction with other healthcare professionals to coordinate treatment plans and care.

Essential Functions
  1. Provide therapy evaluations, treatment planning, interventions, and discharge planning for a full patient caseload in acute care, skilled facility, acute rehab, outpatient settings as appropriate.
  2. Plan, organize, and deliver treatment programs to help clients/patients recover and rehabilitate.
  3. Utilize tools and techniques to administer safe therapeutic interventions and training for patients and families.
  4. Record and document patients' health progress, therapeutic interventions, daily treatments, evaluations, and discharge summaries.
  5. Perform other duties as assigned.
Job Requirements

Bachelor's, Master's, or Doctoral Degree from an accredited Physical Therapy Program.

AND

0-2 years of clinical practice required; 1 year preferred.

Valid PA state license in Physical Therapy. BLS and CPR certification required.
