An established healthcare provider is seeking a dedicated Physical Therapist to join their dynamic team. POSITION: Physical Therapist
SUPERVISOR: Outpatient/Inpatient Senior Clinical Supervisor, Director of Clinical Rehabilitation Services
Engages in timely, creative, and effective use of assistants, aides, and volunteers.
Displays a positive, caring, and responsive attitude towards employees and individuals interacting with the facility.
Demonstrates knowledge of physical therapy techniques, modalities, equipment use, and exercise programs.
Establishes long- and short-term goals and develops comprehensive treatment care plans, adjusting as needed based on patient progress.
Collaborates with patients' families and staff prior to discharge to develop appropriate follow-up plans, including home programs and community resources.
Instructs patients, families, and residents in techniques and home programs, tailoring communication to their age, culture, and educational level, and addressing concerns effectively.
Organizes treatment schedules considering departmental demands.
Instructs on orthosis/prosthetic care and precautions as ordered by physicians.
Provides accurate documentation on evaluations, progress notes, care plans, and other required forms in a timely manner.
Completes insurance documentation promptly.
Leaves clear treatment plans when out of the department and may communicate via phone as needed.
Assists in developing documentation systems to improve efficiency.
Recommends equipment and supplies based on inventory needs.
Attends continuing education programs annually and provides in-service training to staff.
Participates in program development and serves as a clinical instructor for student affiliates, fostering a positive learning environment.
Maintains productivity levels and participates in QA programs, including data collection and analysis.
Travel to contract sites as required.
Contributes to innovative QA studies related to patient care and department operations.
Performs duties ethically and in compliance with laws and facility policies.
Performs other duties as assigned to ensure departmental efficiency.
Degree in Physical Therapy from an accredited program.
Basic entry-level to experienced, including up to three years of work experience or clinical training.
Valid NY Physical Therapy license or eligibility, CPR certification, and ability to obtain a temporary permit if sitting for licensure exam.
