The Physical Therapist evaluates, plans, and implements goal-oriented treatment programs focused on restoring function and preventing further disability caused by trauma, disease, amputation, congenital defects, surgery, or other debilitating illnesses.

Responsibilities

• Participate in committees as requested by supervisor
• Deliver high-quality customer service to residents, families, and visitors
• Model and maintain a safe environment, using correct body mechanics and appropriate treatment techniques
• Establish discharge planning at initial evaluation and update throughout treatment; complete discharge summaries timely
• Assess the need for DME, ensure proper fitting, and educate residents/families on its use and care
• Prepare documentation such as progress notes, reassessments, and discharge summaries in accordance with departmental policies
• Complete re-evaluations and update goals and treatment plans as needed
• Develop rapport and mutual respect with residents and families, providing education tailored to their needs
• Gather necessary evaluation information, including testing and consultations
• Handle special projects and urgent assignments promptly
• Develop individualized treatment plans based on goals and resident needs
• Ensure treatments are completed within productivity standards and documented appropriately
• Order and assess prosthetic/orthotic devices, ensuring proper fit and function, and educate residents and families
• Set measurable short- and long-term goals based on evaluations
• Respect residents' needs and provide evaluations in a respectful manner
• Provide therapy in a safe environment, utilizing proper body mechanics
• Implement treatment programs focused on resident goals, including discharge planning and device utilization
• Perform other duties and projects as assigned, adhering to organizational standards

Qualifications

• Graduate of an APTA-accredited program or equivalent
• Physical ability to perform tasks such as standing, walking, kneeling, climbing, reaching, pushing, pulling, and lifting (20+ lbs)
• Dexterity and eye-hand coordination for activities like typing and manipulation
• Visual and auditory abilities to monitor instruments, recognize characters, and respond to sounds
• Ability to react quickly in emergencies
• Excellent written and verbal communication skills
• Proficiency with computer platforms like Microsoft Office or Google Suite

Licenses and Certifications

• Physical Therapist License
• AHA BLS Healthcare Provider Certification
