Job summary

VNS Health seeks a licensed Physical Therapist to provide home-based rehabilitation services across New York. You will conduct initial assessments, develop personalized treatment plans, and deliver 1:1 therapy in patients’ homes or care facilities, ensuring progress and adjusting plans as needed.

The role emphasizes autonomy, use of AI-assisted documentation, and collaboration with an interdisciplinary team to restore mobility and independence for patients in their daily lives.

Qualifications

• Minimum of one-year physical therapy experience or successful completion of probationary period as a rehab intern (VNS Health).
• Licensure to practice as a Physical Therapist in New York State.
• Valid driver's license required for all areas outside Manhattan.

Qualifications

What You'll Need

Minimum of one-year physical therapy experience or successful completion of probationary period as a VNS Health Rehab Intern.

Licensure to practice as a Physical Therapist in New York State.

Valid driver's license required for all areas outside the borough of Manhattan.

Pay Range per Visit:

If you are applying for the per diem, per visit version of this position, the standard pay range is as follows per visit.

• All areas except the Bronx and the following parts of Brooklyn (Downtown Brooklyn, Williamsburg, and Greenpoint): $70–$75 per visit
• Bronx: Weekday visits – $95 | Weekend visits – $100
• Brooklyn (Downtown Brooklyn, Williamsburg, and Greenpoint only): Weekday visits – $95 | Weekend visits – $100
