Job summary

VNS Health is seeking dedicated Physical Therapists in New York City to provide personalized rehabilitation therapy to patients at home. Join our commitment to helping New Yorkers recover after surgery, illness, or accidents as part of a collaborative care team.

Qualifications

• Minimum of one year physical therapy experience or completion of VNS Health Rehab Intern period.
• Current New York State License in Physical Therapy required.
• Valid driver's license or NYS Non-Driver photo ID may be necessary.

Responsibilities

• Practice independently in the community as part of a care team.
• Conduct comprehensive patient assessments and develop treatment plans.
• Deliver personalized rehabilitation therapy in homes or care facilities.
• Evaluate and adjust patient care plans as needed.

Compensation

$85,000.00 - $127,500.00 Annual

What You Will Do

• Practice independently in the community as part of an interdisciplinary care team.
• Conduct comprehensive assessment of patients by determining patient needs, developing courses of action including treatment plan and outcomes.
• Deliver personalized rehabilitation therapy to patients in their home or care facilities.
• Constantly evaluate evolving patient needs and respond with plan of care adjustments.

Qualifications

• Minimum of one-year physical therapy experience or successful completion of probationary period as a VNS Health Rehab Intern
• New York State License and current registration in Physical Therapy
• Valid driver’s license or NYS Non‑Driver photo ID card may be required

Pay Range per Visit

• If you are applying to the per diem per visit version of this job, the hiring range is as follows: $70‑$75 per visit.
