Job description

Professional member of patient’s treatment who evaluates, assesses, and delivers services according to a written plan of treatment established by a physician. Provides supervision for all services rendered by Physical Therapy Assistants. Evaluations are performed by peers.

Responsibilities of the Registered Physical Therapist include, but are not limited to:
  1. Provide comprehensive assessment, which may include OASIS data elements, to evaluate patient’s physical functioning and need for physical therapy services.
  2. Discuss evaluation with physician, seek input from patient/caregiver, and establish a plan of care to help the patient meet maximum rehabilitation potential.
  3. Carry out prescribed treatments and supervise PTAs in executing the PT program.
  4. Establish and instruct in home exercise programs (HEP); document instructions on visit notes.
  5. Observe and report pertinent changes in the patient’s functioning to the physician and communicate these changes to other team members involved in care.
  6. Maintain accurate documentation of all services as per agency policy.
  7. Conduct oneself professionally at all times and in all situations.
  8. Provide the agency with required evidence, including license/registration copies, for experience verification.
  9. Accept assignments only for which qualified.
  10. Comply with all applicable state, federal, local, and agency rules, regulations, policies, and procedures.
  11. Maintain confidentiality of all patient and agency information.
  12. Adhere to policies and procedures, including HIPAA compliance.
  13. Respect patient rights and cultural beliefs.

Required Qualifications

  • Master's Degree in Physical Therapy (or grandfathered licensure for those who obtained licensure prior to the master's requirement)
  • Current license to practice physical therapy, with ongoing continuing education (24 hours annually)
  • Current CPR certification
  • Participation in Humana's Tuberculosis (TB) screening program
  • Valid state driver’s license and proof of personal vehicle liability insurance (minimum limits: 100,000/300,000/100,000)
  • ACHA Level II Clearance
  • At least one year of relevant physical therapy experience
  • Excellent verbal and written communication skills
  • Sensitivity to cultural diversity and ability to accommodate diverse populations
  • Strong customer service skills
  • Passion for contributing to an organization focused on improving consumer experiences

Preferred Qualifications

  • Doctorate degree in Physical Therapy
  • Bilingual abilities
Additional Information

Scheduled Weekly Hours: 1

Pay Range: $85,400 - $117,500 annually (may vary based on location and experience)
