Qualifications

Duties and Responsibilities:

• Assess referred patients requiring rehabilitation services.
• Provide direct patient care to evaluate medical condition, functional capabilities, limitations, and rehabilitation potential.
• Establish and administer treatment programs with goals tailored to the patient's capacity and under Physician's guidance.
• Set realistic and achievable goals for patients.
• Document patient conditions, educate patients and families in appropriate physical therapy methods.
• Complete evaluations, weekly progress notes, recertifications, and discharge documentation.
• Collaborate with all disciplines for patient-centered care.
• Perform additional duties as assigned by the Director of Rehabilitation.
• Maintain confidentiality and comply with HIPAA and Residents' Rights regulations.

Requirements/Qualifications:

• Graduated from an accredited Physical Therapy program.
• Valid professional license and registration in NJ.
• Experience in long-term or geriatric care preferred.
• Experience with Case Mix Index and PPS scheduling preferred.
