Job description

Paradigm Rehabilitation is seeking passionate Physical Therapists to join our multidisciplinary team and provide high-quality care for pediatric and geriatric patients across NYC boroughs, Albany, Schenectady, Buffalo, Rochester, and Westchester.

Position Overview

As a Physical Therapist, you will conduct home visits, assess patients' mobility, strength, and overall physical function, and develop personalized treatment plans to improve rehabilitation outcomes. Requirements

• Graduate from an accredited Physical Therapy program
• Unencumbered NY Physical Therapy license and registration
• Experience with home visit evaluations and treatment planning
• Basic computer skills and proficiency in EMR documentation (Hello Note preferred but not required)
• Interest in working with pediatric and geriatric populations (preferred)
• Excellent communication skills
• Maintain CPR certification and safety protocols
• Manage a caseload of 4-5 patients (preferred)
