Job summary

NorthLakes Community Clinic is seeking a Physical Therapist to plan and administer treatment plans that promote overall fitness and health for patients. Candidates must reside within NorthLakes' service area and maintain residency throughout employment.

A $7,500 sign-on bonus is offered for eligible positions. Responsibilities include evaluating patients, diagnosing movement dysfunction, creating care plans, educating patients on exercises, and collaborating with other providers.

Qualifications

  • Master's degree in Physical Therapy from an accredited program.
  • Wisconsin Physical Therapy license active.
  • Successful passing of the National Physical Therapy Examination.

Responsibilities

  • Consult with and evaluate the patient to determine goals and needs.
  • Diagnose and treat movement dysfunction.
  • Create plans of care and document patient records per regulatory and accrediting requirements.
  • Provide therapy education and use of adaptive equipment.
  • Provide hands-on therapy (massage, thermotherapy, dry needling) as applicable.
  • Collaborate with other providers including therapists, surgeons, and the primary care provider.
  • Keep equipment clean and safe; perform other duties as assigned.

Join a team that believes everyone deserves care.Candidates must reside within NorthLakes' service area at the time of hire and maintain residency throughout employment.*** $7,500.00 Sign-On Bonus ***Responsible for planning and administering physical therapy treatment plans designed to promote overall fitness and health of patients.Essential Job FunctionsEssential Functions of the Job: [Other duties may be assigned.]Consult with and evaluate the patient to determine patient goals and needs.Diagnose and treat movement dysfunction.Create plans of care and document patient records in accordance with regulatory, licensing, payor, and accrediting requirements.Provide education on therapy exercise techniques and use of adaptive equipment such as walkers or wheelchairs.Provide hands-on therapy such as massage, thermotherapy, dry needling, etc. as applicable.Work collaboratively with other providers including other therapists, surgeons, and the patient’s primary care provider.Clean and maintain equipment to ensure clean and safe work environment.Other duties as assigned.Qualifications and Education RequirementsMinimum Level of Completed Education: Masters Degree in Physical Therapy from an accredited physical therapy schoolPrior years of experience in similar role: 1-2Experience with systems, computer applications/software: None required.
