Job summary

NorthLakes Community Clinic in Wisconsin seeks a licensed Physical Therapist to plan and administer treatment plans promoting patient fitness and health. Candidates must reside within NorthLakes' service area at hire and throughout employment.

Responsibilities include evaluating patients, diagnosing movement dysfunction, creating care plans, documenting records, and providing education on exercises and adaptive equipment.

Qualifications

  • Masters degree in Physical Therapy from an accredited program.
  • Experience with physical therapy evaluation and plan of care.
  • Active Wisconsin Physical Therapy license and passing NPTES.

Responsibilities

  • Consult with and evaluate the patient to determine goals and needs.
  • Diagnose and treat movement dysfunction.
  • Create plans of care and document patient records in accordance with regulatory requirements.
  • Provide education on therapy techniques and adaptive equipment.
  • Provide hands-on therapy such as massage, thermotherapy, dry needling as applicable.
  • Work collaboratively with other providers including surgeons and the patient’s primary care provider.
  • Clean and maintain equipment to ensure a clean and safe work environment.
  • Other duties as assigned.
