Job summary

Ashland seeks a licensed Physical Therapist to evaluate patients within 24 hours of referral, develop and obtain approval for therapy plans, and treat patients per physician guidance. You will supervise PT assistants, train staff, and ensure accurate record-keeping in a collaborative healthcare setting.

Responsibilities include discharge planning, family education, and coordinating equipment for patient mobility, while maintaining strong professional relationships with the care team.

Qualifications

• Bachelor's degree in Physical Therapy; advanced degrees preferred.
• Proficient computer skills for patient data entry.
• Ability to read and apply policies and procedures.
• Strong numerical skills for documentation and measurements.
• Strong problem solving and analytical abilities.

Responsibilities

• Evaluate patients within 24 hours of referral.
• Develop and obtain approval for treatment plans from referring physician.
• Treat patients per the physician plan.
• Train Restorative Aides with nursing staff.
• Supervise PT assistants in patient care per state act.
• Communicate progress with supervisor and health team.
• Participate in care conferences and reviews as needed.
• Assist in staff in-services and training.
• Record evaluations and daily/weekly notes per PT Board rules.
• Document treatment changes per policy.
• Instruct families for discharge prep and home program.
• Participate in discharge planning.
• Recommend durable medical equipment to aid mobility.
• Report equipment issues to keep tools functional.
• Maintain positive relations with patients, families, and staff.
