Job summary

LiveWell Homecare Agency is seeking a Physical Therapist to evaluate and treat patients predominantly in Rehab and LTAC hospitals. You will develop and implement individualized treatment plans, educate patients and families, and document progress in the EMR while collaborating with physicians, vendors, and the care team.

Responsibilities include preparing equipment orders, mentoring interns, and participating in hospital-specific skills assessments.

Qualifications

• Bachelor's degree in Physical Therapy; Doctoral degree preferred.
• At least 1 year of experience in Rehab, SNF, LTACH, or Acute settings.
• GA PT license or eligibility and BLS certification.
• Basic computer literacy.

• Minimum Bachelor's Degree in Physical Therapy; Doctoral Degree preferred
• At least 1 year of experience in Rehab, SNF, LTACH, or Acute settings
• GA PT license or eligibility and BLS certification
• Basic computer literacy

Additional Duties:

Contribute as part of an interdisciplinary team, serve as a Clinical Instructor/mentor, and assist in hospital-specific skills assessments.
