Job summary

Cane Ridge Post-Acute and Rehabilitation Center in Antioch, TN seeks a skilled Occupational Therapist to assess residents, develop individualized treatment plans, and maximize independence in daily activities.

This role includes supervising COTAs, documenting progress in the medical record, and educating residents, families, and staff on ADLs and safety. You will collaborate with an interdisciplinary team to support care planning and discharge decisions.

Qualifications

• Bachelor’s degree in OT from an accredited program.
• Current, active OT license in Tennessee.
• Experience in geriatric settings and SNF care preferred.

Responsibilities

• Perform OT evaluations and re-evaluations per standards.
• Develop and implement individualized ADL-focused treatment plans.
• Supervise COTAs in plan implementation.
• Complete timely documentation in medical records and therapy system.
• Educate residents, families, and staff on adaptive techniques and safety.
• Collaborate with the interdisciplinary team on care planning and discharge.

Position Summary

The Occupational Therapist (OT) provides skilled therapy services to residents, including evaluation, development, and implementation of treatment plans to maximize independence and participation in daily activities.

Responsibilities

• Perform occupational therapy evaluations and re-evaluations in accordance with professional and regulatory standards.
• Develop and implement individualized treatment plans focused on ADLs, functional mobility, and cognitive/perceptual skills.
• Supervise Certified Occupational Therapy Assistants (COTAs) in the implementation of treatment plans.
• Complete timely and accurate documentation in the medical record and therapy system.
• Educate residents, families, and staff on adaptive techniques, equipment use, and safety.
• Collaborate with the interdisciplinary team on care planning and discharge recommendations.

Qualifications

• Bachelor’s degree or higher in occupational therapy from an accredited program.
• Current, unencumbered OT license in the state of Tennessee.
