Job summary

Endeavor Health is seeking a Physical Therapist for Outpatient services at Glenview, IL. Per diem, days, with flexible hours including evenings and rotating shifts. The position involves evaluation, treatment, and documentation for physician-referred patients at Glenbrook Hospital in Glenview.

Licensed or eligible IL PTs with BLS/CPR are encouraged to apply. Per diem pay includes premium rates for on-call and holidays and opportunities for annual increases based on performance.

Qualifications

• Licsensed or eligible for IL licensure as a Physical Therapist.
• Graduation from a Physical Therapy program approved by relevant accrediting bodies (APTA/AMA).
• BLS/CPR certification required by American Heart Association.

Responsibilities

• Provide evaluation, treatment and follow-up services for physician-referred patients.
• Prepare and maintain patient records including evaluations, progress notes and discharge summaries.
• Participate in clinical supervision of students.

Hourly Pay Range: $43.38 - $67.24 - The hourly pay rate offered is determined by a candidate's expertise and years of experience, among other factors.

Position: Physical Therapist – Outpatient - Per Diem - Days Location: Glenbrook Hospital, Glenview, IL Per Diem: Hours as needed Hours: Days, evenings, rotating Required Travel: No Holidays: no Flat Rate Pay Rate: $51/hour

What you will do:

• Provide evaluation, treatment and follow-up services to physician referred patients
• Prepare and maintain accurate patient records including evaluations, progress notes, discharge summaries and billing
• Participates in the clinical supervision of students

What you will need:

• License: Licensed or eligible for licensure as a Physical Therapist in the State of IL
• Education: Graduation from a program in physical therapy approved by the Committee on Allied Health Education and Accreditation of The Medical Association or the American Physical Therapy Association or its educational equivalent, or The Council on Medical Education of the American Medical Association and the American Physical Therapy Association
• Certification: BLS/CPR certification from the American Heart Association required
• Experience: Outpatient Orthopedics
• Skills: Various Orthopedic Patients
