Mercyhealth Beloit is seeking a licensed Physical Therapist to provide high-quality outpatient rehabilitation services across Mercyhealth Beloit and Mercyhealth Sports Medicine and Rehabilitation Center—Janesville. You will assess patients, design individualized treatment plans, and document care electronically in a collaborative clinic setting.

The role requires Wisconsin licensure, CPR/BLS within 90 days, and active participation in continuous improvement, teaching, and mentoring of staff

Qualifications

• Graduate of an accredited school of Physical Therapy.
• Licensed as a Physical Therapist in WI.
• Approved CPR or BLS certification required within 90 days.

Responsibilities

• Integrates subjective and objective data to establish a treatment plan.
• Utilizes exercise, physical agents, and manual techniques to address deficits.
• Maintains a thorough record of services provided with appropriate coding and billing.
• Communicates with other disciplines and referring providers to enhance outcomes.
• Performs thorough and timely electronic documentation.
• Participates in PI and CQI processes and staff training.
• Plans and conducts lectures and training programs for staff and community groups.
