Job summary

LiveWell Homecare Agency, LLC is seeking a licensed physical therapist to evaluate patients, develop individualized treatment plans, and implement evidence-based interventions to improve outcomes. You will educate patients, discharge planning, and document care while coordinating scheduling and billing.

Collaboration with physicians, leadership, and a multidisciplinary team is essential. Flexibility to support across departments is required.

Qualifications

• Requires a valid license in the practicing state.
• Bachelor’s, Master’s, or Doctorate degree from an accredited program.
• BLS or Basic Cardiac Life Support certification.
• Strong communication, professionalism, and teamwork skills.

Responsibilities include patient education, discharge planning, documentation, billing, scheduling, and supervising support staff. Collaborates with a multidisciplinary team and communicates effectively with physicians and leadership to ensure cohesive care. May serve as a clinical instructor and participate in departmental committees. Flexibility to work across departments based on patient needs is required.

Qualifications & Skills: Requires a valid license in the practicing state, with additional certifications such as BLS or Basic Cardiac Life Support. A Bachelor’s, Master’s, or Doctorate degree from an accredited program is necessary. Strong communication, professionalism, and teamwork skills are essential.

Work Details: Scheduled for 36 hours weekly, primarily during weekday hours, with occasional weekend rotations. No holiday or call obligations. May have remote or hybrid options.

Compensation & Benefits: Salary range from $71,547.84 to $107,321.76. Benefits include comprehensive health coverage, 401(k), professional development, flexible scheduling, and wellness resources.
