A leading health system is seeking a compassionate and reliable Physical Therapist or Physical Therapist Assistant for part-time or per diem home health visits. The role involves providing care, collaborating with healthcare teams, and ensuring patient satisfaction while maintaining independence and quality of life for clients in their homes.
Part-Time or Per Diem Physical Therapist (PT/PTA) - Home Health Visits
We are looking for a compassionate, reliable PT and PTA to join our team to provide excellent care and who enjoys working with patients in an at-home setting.

Responsibilities:
Provides care and services in accordance with the plan of care.
Assists community members and health field personnel to assess, plan for, and provide needed health and related services.
Documents care provided in an accurate and timely manner.
Provides teaching to clients and families.
Coordinates and collaborates with other members of the health care team, community agencies, and referral sources.
Communicates the plan of care with patients and families and insures understanding of the exercise program prescribed.
Responsive to meeting or exceeding patient/client expectations and promoting a positive image of the company
Adheres to HIPAA laws, maintains patient confidentiality, and adheres to all agency policies and procedures.
Attends continuing education programs.
Other duties assigned by the department manager.
Requirements/Qualifications:
Associate's degree and graduation from an accredited Physical Therapy program.
One year of physical therapy experience preferred.
Possesses and maintains CPR Certification
Possess valid driver's license
Skills/Competencies:
Demonstrates responsibility for self-direction
Demonstrates the ability to learn and utilize computer technology
Effective communication skills
The ability to work in a team-oriented environment as well as independently
Qualifications:
Must be licensed by the State of Massachusetts as a Physical Therapy Assistant.
Job Types: Part-time, Per-diem
Expected hours: 5 – 20 hours per week
