Qualifications

Responsibilities

• Manage a daily caseload averaging 13 patients.
• Provide patient and caregiver education on plans and home programs.

• Complete EMR documentation accurately.

Compensation: $90,000 – $105,000 annually (base salary) + performance-based bonus potential
