Job summary

BAYADA Home Health Care in Vermont is seeking a Physical Therapist to deliver at‑home care and build meaningful patient relationships. New graduates are welcomed into a structured mentorship program with support from a collaborative team and flexible scheduling.

You’ll perform mobility and balance assessments, develop evidence‑based treatment plans, provide gait training, and educate clients and families. Documentation is done in our EMR system.

Qualifications

• Current Vermont PT license (or eligibility).
• Graduation from an accredited PT program.
• Comfort with EMR systems (training provided).

Responsibilities

• Perform mobility, strength, balance, and safety assessments.
• Develop individualized, evidence-based treatment plans.
• Provide gait training, manual therapy, balance work, and therapeutic exercise.
• Implement skin and wound-prevention strategies.
• Educate clients and families to promote long‑term success.
• Document care using our intuitive EMR system.

Compensation & Schedule

Base Pay, depending upon qualifications and subject to negotiation:

$57.00-$59.00 per point
