Tender Touch Rehab Services LLC is seeking a Licensed Physical Therapist in New Jersey. This full-time position is set in a Skilled Nursing Facility where you will evaluate and treat patients, creating personalized care plans.

Qualifications

• Must have a degree from an APTA-accredited Physical Therapy program (BS, MS, or DPT).
• Current license as Physical Therapist in New Jersey is required.
• A minimum of one year of experience in a Skilled Nursing Facility (SNF) is preferred.

Responsibilities

• Evaluate and treat patients with mobility, strength, and balance impairments.
• Create personalized care plans tailored to individual patient needs.
• Collaborate with team members to ensure quality patient outcomes.
• Maintain necessary documentation to comply with regulatory standards.
• Ensure physician orders are obtained and maintained up to date.
