Job summary

Tender Touch Rehab Services LLC is seeking a licensed Physical Therapist for full-time work in a Skilled Nursing Facility/Long-Term Care setting in Danbury, CT. You will evaluate and treat patients, develop personalized care plans, and collaborate with a multidisciplinary team to achieve optimal outcomes.

Qualifications

• Candidate must be licensed Physical Therapist in the state of practice.
• New grads welcome; mentorship available.
• Degree from an APTA-accredited PT program (BS, MS, or DPT).

Responsibilities

• Evaluate and treat patients with mobility, strength, balance, and functional impairments.
• Create and implement personalized care plans.
• Collaborate with team members to ensure quality outcomes.
• Maintain documentation to meet all regulatory requirements.
• Ensure all required physician orders are obtained and up-to-date.
