Tender Touch Rehab Services is hiring a Multi-Site Physical Therapist - Full-Time to join our growing team. In SNF/Long-Term Care settings, you’ll travel between facilities and coordinate care with a supportive group of clinicians.

Qualifications

• Degree from an APTA-accredited Physical Therapy program (BS, MS, or DPT).
• Current license as Physical Therapist in the state of practice.
• Strong communication and organizational skills.
• Minimum one year SNF experience and willingness to travel to multiple facilities.

Responsibilities

• Evaluate and treat patients with mobility, strength, balance, and functional impairments
• Create and implement personalized care plans
• Collaborate with team members to ensure quality outcomes
• Maintain documentation to meet regulatory requirements
• Ensure all required physician orders are obtained and up to date
