Job summary

Responsibilities

• Consult with patients to learn about their physical condition and symptoms; review medical history and referrals.
• Diagnose movement dysfunction by listening to patients and observing them as they move.
• Develop individualized treatment plans.
• Coach patients and teach therapeutic massage to ease pain and promote healing.
• Maintain patient records by tracking goals.
• Utilize standardized assessments and evidence-based practice to support clinical interventions.
• Assess patients’ progress and adjust treatments in the clinical record per regulations.
• Evaluate treatment outcomes and modify services accordingly; refer as needed.
• Provide consultation to patients, families, caregivers and other service providers.
• Communicate progress, barriers and plans with supervisors and health team members.
• Provide comprehensive discharge summaries per regulatory requirements.
• Document all information in electronic systems and maintain records per policy.
• Participate in trainings, in-services and conferences.
• Able to supervise and support PT assistants and students.
• Adhere to company policies and code of conduct.
