Job summary

People Well Staffing is seeking a Physical Therapist to join a collaborative SNF team in Memphis, TN. You will evaluate patients, develop individualized plans of care based on functional goals, and provide evidence-based therapy to improve mobility, strength, balance, safety, function, and independence.

You will monitor progress, educate patients and families, document findings, and collaborate with physicians, nursing staff, and other rehabilitation professionals to optimize patient outcomes.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Active TN Physical Therapist license in good standing, or eligibility for licensure.
• Current BLS/CPR certification as required by the employer.
• Strong clinical reasoning, communication, documentation, and teamwork skills.
• Experience in the applicable setting preferred; qualified clinicians at varying experience levels are encouraged to apply.

Responsibilities

• Evaluate patients and develop individualized plans of care based on functional goals.
• Provide evidence-based therapeutic exercise, mobility, balance, gait, and functional training.
• Monitor patient progress and modify treatment plans as clinically appropriate.
• Educate patients, families, and caregivers on safety and home programs.
• Complete accurate and timely clinical documentation.
• Collaborate with physicians, nursing, rehabilitation professionals, and the interdisciplinary team.
