Job summary

Enhance Rehabilitation in Wisconsin is seeking qualified Physical Therapists to join our team in SNF settings. You will evaluate and treat patients, develop plans of care, and set measurable goals, delivering high-quality, patient-centered care.

You will collaborate with facility staff, supervise Physical Therapist Assistants, document evaluations and progress notes, and communicate progress and outcomes to patients and families.

Responsibilities

• Provide excellent patient care in SNF setting.
• Deliver excellent customer service to patients and families.
• Collaborate with facility staff to ensure quality care.
• Evaluate new patients, develop care plans, and set functional goals.
• Treat patients, address concerns and educate on injuries and outcomes.
• Supervise Physical Therapist Assistants.
• Re-evaluate progress, revise plans, and reset goals.
• Complete timely documentation, including evaluations, re-evaluations, discharges and daily progress notes.
