Qualifications

• Licensed Physical Therapist in New York or eligible.
• Ability to evaluate a patient’s PT needs and develop an appropriate treatment plan.
• Provide care per physician care plan and policy.
• Experience instructing family members, PTAs, Home Health Aides, and other caregivers.

Responsibilities

• Evaluate patients and develop personalized PT plans.
• Deliver treatments aligned with physician care plans.
• Educate caregivers and family members on PT techniques.
