Centra Healthcare seeks a licensed Physical Therapist Manager (PT) for an outpatient facility in Rocklin, CA. This is a direct hire, permanent role with a days schedule supporting a growing team.

The ideal candidate will lead clinical operations, develop care plans for PT patients, provide skilled therapy per physician orders, and collaborate with multidisciplinary colleagues to achieve patient goals.

Qualifications

• Must be a licensed Physical Therapist in California.
• Current CPR Certification.
• Experience in outpatient settings preferred.
• Develop a care plan for each PT patient and provide therapy per physician orders.
• Collaborate with multidisciplinary teams to meet goals.

Job Description:

A great Outpatient Facility is looking for an experienced or newly graduated Physical Therapist / PT Manager to join their growing team!

• This is a direct hire / 'perm' position.
• Days schedule.
• This role assumes responsibility and accountability for incorporating the vision, values, mission and critical goals of the organization into their job performance.

Requirements include:

• Must have graduated from an accredited school.
• Current state license as a Registered Physical Therapist / PT or proactively in the process of the application process for current state licensure as a Registered Physical Therapist / PT.
• Current CPR certification.
• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services / interventions in accordance with physician orders.
• Assess patient needs, plans for, evaluates and modifies care to meet goals of physical therapy interventions.
• Collaborates with all disciplines to plan and evaluate team goals for each patient.
