Job summary

Centra Healthcare Solutions is seeking a licensed Physical Therapist (PT) for an outpatient role in San Leandro, CA. This perm/direct-hire position requires an accredited PT program graduate with active licensure or in-process licensure and CPR certification.

You will develop care plans, provide skilled PT interventions, and collaborate with cross-disciplinary teams to meet patient goals in a dynamic outpatient setting.

Qualifications

• Must have graduated from an accredited physical therapy program.
• Current state license as a Registered Physical Therapist (PT) or active licensure in progress.
• Current CPR certification.
• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services/interventions per physician orders.
• Assess patient needs and adapt care to meet goals.
• Collaborate with all disciplines to plan and evaluate team goals.

Job Description:

A great Outpatient Facility is looking for an experienced or newly graduated Physical Therapist / PT to join their growing team!

• This is a direct hire / 'perm' position.
• Days schedule.
• This role assumes responsibility and accountability for incorporating the vision, values, mission and critical goals of the organization into their job performance.

Requirements include:

• Must have graduated from an accredited school.
• Current state license as a Registered Physical Therapist / PT or proactively in the process of the application process for current state licensure as a Registered Physical Therapist / PT.
• Current CPR certification.
• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services / interventions in accordance with physician orders.
• Assesses patient needs, plans for, evaluates and modifies care to meet goals of physical therapy interventions.
• Collaborates with all disciplines to plan and evaluate team goals for each patient.
