Job summary

Progressive Rehab Solutions in Tyler, TX is seeking a Physical Therapist to evaluate, plan, and implement individualized therapy programs in long-term care or related settings to improve mobility and function.

You will lead treatment planning, provide gait and transfer training, document outcomes, and collaborate with an interdisciplinary team. A DPT or equivalent and active TX licensure are required; 1 year of experience is preferred.

Qualifications

• Doctor of Physical Therapy (DPT) or equivalent required.
• Current and valid state license to practice Physical Therapy (Texas) required.
• One year of experience in long-term care or rehabilitation preferred.

Responsibilities

• Evaluate patients' condition and plan individualized therapy.
• Provide gait training, transfer training and therapeutic exercises.
• Document evaluations, progress notes, and discharge summaries per regulations.
• Collaborate with interdisciplinary team and supervise PTAs as applicable.
• Educate patients and caregivers on exercises, safety and home programs.
