Job summary

Vertis Therapy is seeking a compassionate Full-Time or PRN Physical Therapist to join our rehabilitation team at an assisted living and memory care community in Naples, Florida. This role kicks off a new therapy program launching mid-October with a flexible schedule to fit your availability.

You will develop individualized treatment plans, provide skilled PT services, monitor resident progress, and supervise PTAs.

Qualifications

• Degree in Physical Therapy from an accredited program.
• Valid state license as a Physical Therapist or eligibility.
• Strong clinical assessment and treatment planning skills.
• Excellent communication and interpersonal abilities.
• Ability to work independently and within a multidisciplinary team.
• Proficiency in documenting care in an EMR system.

Responsibilities

• Resident assessment and treatment to improve mobility and safety.
• Timely and accurate documentation of patient care.
• Supervision of Physical Therapy Assistants as appropriate.
• Participate in or lead team development meetings.
• Lead or participate in wellness programs for residents.
