Job summary

Confluence Health seeks a licensed Physical Therapist to diagnose disorders and provide therapy for patients with musculoskeletal or neurological impairments. You will evaluate patients, develop treatment plans, administer therapy, and plan for discharge.

Position reports to Rehab Services Manager. Must maintain BLS certification and collaborate with the care team to ensure evidence-based, patient-centered care across inpatient and outpatient settings.

Qualifications

  • Active or interim WA State Physical Therapist License, required by start date.
  • One year of outpatient physical therapy experience preferred.

Responsibilities

  • Perform initial evaluation including history of prior level of function, subjective information, objective tests and measures, assessment and plan of care.
  • Incorporate evidence based clinical decision making to select and utilize appropriate patient interventions.
  • Follow best practice for clinical documentation.
  • Maintain patient safety, modesty and privacy/confidentiality at all times.
  • Provide education to patients, caregivers and other staff members.
  • Provide supervision and guidance to Physical Therapy Assistants and rehabilitation aides.
  • Collaborate with physicians, case managers, nurses and other therapy staff, including discharge planning.
  • Screen for and refer to other disciplines when indicated.
  • Maintain current BLS certification.
  • Other duties as assigned.

A Physical Therapist provides professional services in diagnosing disorders and providing therapy for patients with musculoskeletal or neurological impairments. This includes evaluation of all assigned/scheduled patients, the development of treatment plans, administration and implementation of the treatment program, and appropriate discharge planning.

Essential Functions
  1. Perform initial evaluation including history of prior level of function, subjective information, objective tests and measures, assessment (including discharge disposition recommendation in acute setting), and establishment of plan of care and goals for future treatments.
  2. Incorporate evidence based clinical decision making to select and utilize appropriate patient interventions in accordance with plan of care, and modify plan of care as appropriate based on patient progress/response to interventions.
  3. Follow best practice for clinical documentation.
  4. Maintain patient safety, modesty and privacy/confidentiality at all times.
  5. Provide education to patients, caregivers and other staff members as appropriate to include adaptive equipment and recommended safety precautions.
  6. Provide supervision and guidance as needed to Physical Therapy Assistants and rehabilitation aides.
  7. Collaborate and communicate effectively with other team members including physicians, case managers, nurses, and other therapy staff, including participation in discharge planning in inpatient settings.
  8. Screen for and make referrals to other disciplines when indicated.
  9. Required to maintain current BLS certification.
  10. Other duties as assigned.
