Job summary

Confluence Health is seeking an Occupational Therapist to evaluate and treat patients under the Rehab Manager. External applicants are preferred, with sign-on/retention bonuses up to $15,000. The role involves chart reviews, initial evaluations, plan of care, and home safety assessments.

You will collaborate with physicians, nurses, and case managers, provide patient education, and supervise rehab staff while upholding standards of care and confidentiality.

Qualifications

Active WA State OT license or active license in progress with expectation to obtain within the first year.
Graduate from an accredited occupational therapy program.
Completion of clinical internship.

Responsibilities

Conduct thorough chart review and screening for appropriateness of OT evaluation.
Perform initial evaluation with patient history, tests and measures, plan of care and goals.
Apply evidence-based interventions and modify plan of care by patient progress.
Identify adaptive equipment needs and educate patients and caregivers.
Maintain clear documentation and ensure safe, coordinated care.
Ensure patient safety, privacy and confidentiality at all times.
Provide education to patients, caregivers and staff.
Supervise and guide OT Assistants and rehab aides as needed.
Collaborate with physicians, case managers and nurses during discharge planning.

Summary

Under the direction of the Rehab Manager, evaluates, plans and administers medically prescribed occupational therapy treatment to patients.

Position Reports To: Rehab Manager

Essential Functions

Conduct thorough chart review and screening for appropriateness of patient for OT evaluation.
Perform initial evaluation including history of prior level of function, subjective information, objective tests and measures, assessment (including discharge disposition recommendation in acute setting), and establishment of plan of care and goals for future treatments. Perform home safety evaluations.
Incorporate evidence based clinical decision making to select and utilize appropriate patient interventions in accordance with plan of care, and modify plan of care as appropriate based on patient progress/response to interventions.
Identify patient’s adaptive equipment needs, instruct patients and caregivers in safe and correct use of equipment, and collaborate with other disciplines/equipment suppliers to ensure patient’s equipment needs are met.
Maintain clear documentation records and use clinical judgment to determine when additional communication beyond standard documentation is required for safe and coordinated patient care.
Maintain patient safety, modesty, and privacy/confidentiality at all times.
Provide education to patients, caregivers and other staff members as appropriate.
Provide supervision and guidance as needed to Certified Occupational Therapist Assistants and rehabilitation aides.
Collaborate and communicate effectively with other team members including physicians, case managers, nurses, and othertherapy staff, including participation in discharge planning in inpatient settings.
Screen for and make referrals to other disciplines when indicated, including mandatory reporting in cases of suspected abuse/neglect.
Meet required licensure and continuing education requirements.
Required to maintain active Basic Life Support (CPR) certification.
Other duties as assigned.

Qualifications

Required:

Active Washington State Occupational Therapist License or Active Occupational limited permit with expectation to obtain Active Washington Occupational Therapist license in the first year.
Graduated from an accredited occupational therapy program.
Completion of clinical internship.

Desired:

Two years of occupational therapy experience.

Physical/Sensory Demands

O = Occasional, represents 1 to 25% or up to 30 minutes in a 2 hour workday. F = Frequent, represents 26 to 50% or up to 1 hour of a 2 hour workday. C = Continuous, represents 51% to 100% or up to 2 hours of a 2 hour workday.
