Job summary

Evolve Therapy Services is hiring a Physical Therapist for an in-house Skilled Nursing Facility located in Hadley, MA. This full-time role offers a $3,000 sign-on bonus, with additional Part Time or PRN options available.

Responsibilities include evaluating residents, developing treatment plans, documenting in NetHealth EMR, and collaborating with an interdisciplinary team to ensure high-quality resident care. Requires a BS in Physical Therapy and an active PT license.

Qualifications

• Bachelor of Science degree in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents per standards and procedures.
• Document and bill services per CMS and company requirements.
• Develop treatment plans with short- and long-term goals.
• Screen residents to determine need for intervention.
• Communicate findings to residents, families, and interdisciplinary team.
