Job summary

Evolve Therapy Services seeks a licensed Physical Therapist for an in-house Skilled Nursing Facility in Sutherland, Nebraska. This PRN position focuses on evaluating residents, creating treatment plans, and delivering high-quality therapy aligned with CMS guidelines.

You will work within an interdisciplinary team to evaluate, treat, document, and adjust care plans while maintaining licensure and pursuing ongoing professional development.

Qualifications

Responsibilities

• Provide physical therapy to residents according to established standards and department procedures.
• Complete documentation and billing in NetHealth EMR per CMS expectations.
• Develop and adjust treatment plans and goals with Resident and care team.
• Screen residents to determine need for intervention and plan treatments.
• Communicate findings and recommendations to residents, families, and interdisciplinary team.
• Complete all duties to ensure high-quality resident care.
