Job summary

Evolve Therapy Services is seeking a compassionate Physical Therapist for an in-house Skilled Nursing Facility in Greendale, WI. This part-time/PRN position focuses on delivering high-quality resident care.

You will plan and direct PT services, evaluate residents, develop treatment plans, document per CMS in NetHealth EMR, and collaborate with an interdisciplinary team. Ongoing continuing education and licensure maintenance required.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents per standards and procedures.
• Adhere to documentation and billing procedures.
• Evaluate residents to inform treatment planning and implementation.
• Develop treatment plans with short and long-term goals.
• Communicate evaluation findings to residents, families, and the care team.
