Job summary

Baptist Health Rehab Services seeks a licensed Physical Therapist to evaluate and treat patients, develop treatment plans, and set goals. You will educate patients and families and ensure documentation complies with regulations.

The PT will also supervise students and Rehab Services staff while supporting departmental goals. Participation in community education and interdepartmental collaboration is expected, with attention to patients across ages and conditions.

Qualifications

• Bachelor's Degree in Physical Therapy
• Current license by the KY Board of Physical Therapy
• BLS within 30 days from date of hire

Responsibilities

• Completes appropriate chart / medical record review.
• Follows plan of treatment appropriately, notifies referring provider of change in status and need for modifications to treatment plan or goals.
• Plans treatment appropriately, including input from patient.
• Receives and implements therapy orders.
• Completes evaluation form, treatment notes, progress reports, and D/C summaries in a timely manner, and appropriately submits to referring provider.
• Supervises/delegates/directs activities of students and Rehab Services Technicians.
