Qualifications

• Bachelor’s degree in Physical Therapy is required.

• Current CPR card is required.
• Illinois eligible or licensed Physical Therapist; CPR certification is required.

Responsibilities

• Evaluate and treat patients with a wide variety of impairments.
• Collaborate with physicians and other team members to coordinate care.
• Provide and coordinate comprehensive care using evaluated plans and treatments.
• Maintain timely medical and education records and communicate with patients and families.
• Support a clinical ladder program to advance skills and knowledge.
