Legacy Healthcare Services, Inc. seeks a compassionate Physical Therapist (PT) to join our team for a hybrid role combining on-site facility care with in-home therapy through the Better at Home program across Rowan and Mecklenburg Counties.

You will provide private, functional-based 1-on-1 sessions, develop individualized care plans, and deliver high-quality therapy while collaborating with an interdisciplinary team and supervising PTAs as required by state regulations.

Qualifications

• Degree in Physical Therapy from an accredited program.
• Current Physical Therapy license in the state of North Carolina (or eligibility to obtain prior to start date).
• Excellent clinical, interpersonal, and fiscal management skills.
• Proficiency with computers and electronic documentation systems.

Responsibilities

• Assess, plan, and deliver high-quality physical therapy services according to physician orders and the State Physical Therapy Practice Act.
• Provide individualized, 1-on-1 rehabilitation treatments on-site and in patient homes.
• Evaluate home environments, gait, mobility, and ROM to build tailored care plans.
• Educate patients, families, and caregivers on home exercises and safe mobility strategies.
• Coordinate integrated care plans with multidisciplinary teams and identify referrals as needed.
• Maintain progress summaries and supervise PTAs in compliance with state regulations.

Legacy Healthcare Services is searching for an experienced, motivated, and compassionate Physical Therapist (PT) to join our team. This unique hybrid opportunity combines on-site facility care at Oak Park Retirement with personalized home-based therapy through our Better at Home program across Rowan and Mecklenburg Counties.

In this role, you will work directly with senior adults in both a vibrant community setting and in the comfortable environment of their own homes. With private, functional-based, 1-on-1 treatment sessions and no group setting constraints, you will have the autonomy to individualize care plans, build deep clinical relationships, and help patients live their best lives with purpose, safety, and joy.

Key Responsibilities

• Comprehensive Clinical Care: Assess, plan, and deliver high-quality physical therapy services according to physician orders and the State Physical Therapy Practice Act.
• Dual-Setting Treatment: Provide individualized, 1-on-1 rehabilitation treatments on-site at Covenant Village as well as in patient homes throughout Rowan and Mecklenburg Counties.
• Environmental & Functional Assessments: Evaluate home environments, muscle strength, gait, mobility, and ROM potential to build tailored rehabilitative care plans.
• Caregiver & Family Guidance: Educate patients, families, and caregivers on home exercises, safe mobility strategies, and the proper use of therapeutic appliances.
• Interdisciplinary Collaboration: Participate in clinical conferences, coordinate integrated care plans with multidisciplinary teams, and identify referrals for additional home health (PCA) services as needed.
• Documentation & Leadership: Maintain progress summaries based on goal attainment and supervise Physical Therapist Assistants (PTAs) in compliance with state regulations and company standards.
