Job summary

Legacy Healthcare Services in Greensboro, NC seeks an experienced Physical Therapist to join our team. This hybrid role combines on-site care at Heritage Greens with personalized home-based therapy across Guilford County, empowering seniors with individualized plans and meaningful patient relationships.

You will deliver 1-on-1 sessions, collaborate with a multidisciplinary team, and ensure complete documentation while supervising PTAs as required.

Qualifications

• Current Physical Therapy license in the state of North Carolina (or ability to obtain prior to start date).
• Degree in Physical Therapy from an accredited program.

Responsibilities

• Evaluate, assess, and provide physical therapy services in accordance with physician plans of care and the State Physical Therapy Practice Act.
• Deliver high-quality, 1-on-1 rehabilitation services both on-site at Heritage Greens and directly in patients' homes throughout Greensboro/Guilford County.
• Evaluate home environments, mobility, gait, muscle strength, and ROM to build tailored rehabilitation programs.
• Instruct patients, family members, and caregivers on therapeutic exercises and safe mobility strategies.
• Participate in clinical conferences and coordinate care plans as part of a multi-disciplinary team.
• Prepare timely progress summaries and supervise PTAs in accordance with state regulations and company policy.

Legacy Healthcare Services is searching for an experienced, motivated, and compassionate Physical Therapist (PT) to join our team in Greensboro, NC. This unique hybrid opportunity combines on-site facility care at Heritage Greenswith personalized home-based therapy through our Better at Home program across Guilford county.

You will work directly with senior adults both in their residential community and in the comfort of their homes, empowering them to lead lives filled with purpose, safety, and joy. With private, functional-based, 1-on-1 treatment sessions, you will have the autonomy to individualize care plans and truly build meaningful relationships with your patients.

Key Responsibilities

• Comprehensive Patient Care: Evaluate, assess, and provide physical therapy services in accordance with physician plans of care and the State Physical Therapy Practice Act.

• Dual-Setting Treatment: Deliver high-quality, 1-on-1 rehabilitation services both on-site at Heritage Greens and directly in patients' homes throughout Greensboro/Guilford County.

• Environment & Mobility Assessment: Evaluate home environments, mobility, gait, muscle strength, and ROM potential to build tailored rehabilitation programs.

• Caregiver Education: Instruct patients, family members, and caregivers on therapeutic appliances, home exercises, and safe mobility strategies.

• Interdisciplinary Collaboration: Participate in clinical conferences and coordinate care plans as part of a multi-disciplinary rehabilitation team. Identify additional patient needs (such as PCA services) and make appropriate referrals.

• Documentation & Supervision: Prepare timely progress summaries and supervise Physical Therapist Assistants (PTAs) in accordance with state regulations and company policy.

Pay Range
