Legacy Healthcare Services, Inc. is seeking an experienced Physical Therapist (PT) to join our Better at Home team in Raleigh, NC. This role involves providing personalized physical therapy services to patients in their homes and contributing to their rehabilitation and quality of life.

Qualifications

• Current Physical Therapy license in the state of practice or ability to obtain it before starting employment.
• Excellent interpersonal, fiscal, and clinical skills.
• Computer proficiency required.

Responsibilities

• Provide physical therapy services to patients according to physician's plan of care.
• Assess therapeutic and functional status to develop physical therapy programs for rehabilitation.
• Evaluate the home environment and assess patients for rehab potential.
• Direct patients and families in the use of therapeutic appliances.
• Initiate and instruct others in physical therapy programs.
• Identify needs for other home health services.
• Prepare and submit progress summaries on goal attainment.
• Supervise PTAs as per regulations.

• Pay Rate:Based on a Per Visit Rate

• Coverage Location:Wake County, NC

Responsibilities

• Provide physical therapy services to patients according to physician's plan of care and as defined in the state Physical Therapy Practice Act.
• Assess and evaluate therapeutic, rehabilitative/functional status to develop, implement and direct physical therapy programs that focus on rehabilitation.
• Evaluate the patient’s home environment, assess patients for muscle strength, mobility, gait, ROM potential for rehab.
• Direct patients and families/caregivers in the use and care of therapeutic appliances.
• Initiate physical therapy program and instruct other personnel and/or family/caregiver members in certain phases of physical therapy with which they may work with a patient.
• Identify patient and family/caregiver needs for other home health (PCA) services and refer as necessary.
• Prepare and submit progress summaries based on the attainment of goals.
• Supervises PTAs according to organization policy and state regulations.
