Legacy Healthcare Services is seeking a Physical Therapist to join our on-site team in a senior living setting. You will provide rehabilitative services to residents, develop and adjust treatment plans, and collaborate in clinical conferences to coordinate plans of care.

The role emphasizes individualized, one-on-one sessions with a focus on functional-based outcomes. Ideal candidates hold a Physical Therapy license in the state of practice and demonstrate strong interpersonal and clinical

Qualifications

• Licensed PT or eligible for licensure in the state of practice.
• Excellent interpersonal and clinical skills.
• Proficiency with computers and electronic records.

Responsibilities

• Provide rehabilitative services to residents as part of a coordinated team.
• Plan initial and subsequent treatment programs based on clinical findings.
• Participate in clinical conferences to coordinate the plan of care for residents.
• Treat residents one-on-one with individualized, functional based treatment sessions.
• Tailor treatment sessions to each patient to maximize outcomes.

Position requirements:

• Degree in area of discipline
• Current Physical Therapy license in state of practice, or ability to obtain prior to beginning employment
• Excellent interpersonal, fiscal, and clinical skills
• Computer proficiency

Pay Range

USD $45.00 - USD $53.00 /Hr.
